Is it possible to retain capitalization of Add Files names in the .iss file (see attached)

It is not currently an option but why would you need to keep capitalization? The entries are the source files and the names never get deployed.

Ah, Unless of course you want the file names to stay capitalized... I know there was a reason to make the file names all lowercase. I will research this.

You must replace your files with these.

InnoScript.exe: C:\Program Files\Randem Systems\InnoScript 10

English (U.S.).ist: C:\Documents and Settings\YOURUSERNAME\Local Settings\Application Data\Randem Systems\InnoScript\Release\Languages

Did it work? I may need to send another file to go along with the exe if it did not.
