CHG:  404 error file extension exceptions can be modified in the "Settings" area.

NEW:  "Added Record only Mode" to not block IP addresses but to only redirect abuser away from the form. This was added so that multiple users of the same IP don't get locked out when 1 user on that same IP abuses the forum. Only that abuser is denid access to the forum. This also helps when a legitimate user makes a mistake, they do not get locked out only redirected.
